Medical Plans Overview

The following provides a summary of the information about the medical plans we offer.

We offer you a choice of medical plans so you can choose the best fit for you. Please take the time to understand how these plans work, the coverage each provides, and how to use them to best meet the needs of you and your family.

Choice of four medical plans

  • UnitedHealthcare PPO HSA 2250/4500
  • UnitedHealthcare EPO HSA 4000/8000
  • Surest Zero-Deductible plan

  • Employee only (yourself)
  • Employee plus spouse (or legally recognized partner)

How the plans work

Each plan has its own set of copays, deductibles, out-of-pocket maximums and coinsurance, and preventive care is covered at 100% when you use an in-network provider.

  • The PPO HSA and EPO HSA are HSA-eligible plans, meaning that you can contribute to a Health Savings Account (HSA) and use that money to pay for eligible health care expenses. For 2025, annual contributions to an HSA can be up to $4,300 for self-only coverage and up to $8,550 for family coverage. (Note: These limits include not only your contributions but also company contributions in the case of the PPO HSA Plan.)
  • For the PPO and PPO HSA plans, you can use in-network and out-of-network providers. Keep in mind, you will typically pay lower costs when you use in-network providers. For the PPO HSA Plan only, the company will contribute to your HSA: $500 when you enroll in individual coverage or $1,000 when you enroll in family coverage.
  • The two HSA plans continue to offer a Core Plus Preventive Drug List, which means you do not have to meet a deductible before filling a prescription for preventive drugs included on the coverage list.
  • The Surest Zero-Deductible plan has a $0 deductible, with copays for each Medical service, treatment and prescription, except for preventive services which are covered at 100%. A key feature of the Surest plan is that you can use the Surest app to find out the cost of services in advance, so you can consider treatment options and compare costs from different providers. When you’re enrolled, you can visit the Surest member page to register and set up your profile, and download the Surest app, as well as check prices, compare treatment options, and find an in-network provider. Information about ID cards:

  • ID cards for the UnitedHealthcare plans are available at myuhc.com and won’t be mailed to homes. Existing cards from the prior plan year are valid; however, new cards will include additional plan information.
  • Surest Zero-Deductible plan enrollees receive a welcome packet with an ID card and have access to a digital ID card through the Surest app or website.
Preventive care

All four medical plans include 100% coverage for in-network preventive care. This includes:

  • Physician office services
    • Routine physical examinations
    • Well baby and well child care
    • Immunizations
  • Lab, X-ray or health screening tests*, including those for:
    • Mammography
    • Colorectal
    • Osteoporosis

* Many types of exams are subject to health status and age/gender guidelines.

Maven: Maternity support and more

Available to employees and spouses/partners enrolled in any AI Fire medical plan, Maven through UHC provides support for all stages of your pregnancy, from preconception/fertilty concerns to the first three months of early parenting. It also provides support for menopausal and midlife health issues.

  • Breastfeeding or bottle feeding
  • Navigating infant sleep
  • Managing your emotional and mental health

Resources available through Maven’s digital family health platform include personalized guidance, education, coaching, virtual classes and more. The program provides 24/7 virtual access to specialists across more than 30 specialties including doulas, mental health professionals, career coaches, and lactation consultants. A dedicated Care Advocate will help you find the right provider for your needs, refer you to the best-in-person support in your area, and be a source of support throughout your journey.

Visits by video 24/7

If covered by a UnitedHealthcare medical plan: When you need care — anytime, day or night — Virtual Visits can be a convenient option. From treating flu and fevers to caring for migraines and allergies, you can video chat with a doctor 24/7.

Here’s how Virtual Visits work and the steps you need to take:

  1. Complete a health profile anytime at www.myuhc.com/virtualvisits.
  2. Request a visit when you are sick.
  3. Get a diagnosis and prescription (if needed) in 20 minutes or less.

Hearing aid coverage

Our medical plans include coverage for hearing aids, to make these devices more accessible and affordable to you. For the PPO 1500/3000 Plan, the PPO HSA 2250/4500 Plan and the EPO HSA 4000/8000 Plan, hearing aid coverage is provided through UnitedHealthcare Hearing. Each of these plans pays up to $2,500 per year for hearing aid benefits, which cover one purchase per hearing-impaired ear (including hearing aid repair and replacement) every three calendar years.

Medical care providers come to YOU!

If covered by a UnitedHealthcare medical plan: DispatchHealth provides on-call medical care directly to your door. Avoid visiting the ER or urgent care center, and instead have trained medical staff come to you — conveniently to your home, office or another location. Care is available seven days a week, including holidays, from 8am to 10pm. In addition, the medical team manages calling in prescriptions you need, communicating with your doctor, and overseeing billing.

Manage your weight

If covered by a UnitedHealthcare medical plan: Nearly seven out of 10 adults are considered overweight or obese. Surest plan members can obtain virtual doctor visits for physical and mental health through Doctor on Demand.

Terms to know

Coinsurance – A set percentage you pay of the cost of the care you receive; for example, 20%. The plan pays the rest.

Copay – A set dollar amount you pay when you receive health care; for example, $30 when you see a specialist.

Deductible – A set amount you must pay out of your pocket before your health insurance starts paying part of the cost, unless a copay applies.

Out-of-pocket maximum – The most you will pay in a calendar year for doctor visits, prescriptions, etc. This “safety net” provides peace of mind for those who have a serious condition or illness. Once a covered family member meets the individual out-of-pocket maximum, the plan will pay the full cost of covered charges for that family member. Charges for all covered family members will continue to count toward the family out-of-pocket maximum. Once the family out-of-pocket maximum is met, the plan will pay the full cost of covered charges for all covered family members. The annual out-of-pocket maximum includes amounts paid toward your deductible.
