What’s New for 2026
For the most part, your benefits will be the same as in 2025, with some important changes:
Medical and Prescription Drug
- Due to rising health care costs, there are increases to our medical plan rates to preserve the financial health and sustainability of our overall benefits plan. AI Fire will cover 70% of the medical cost increases this year and will continue to cover 70% of the overall medical plan costs, but employee contributions will also see some increases. The 2026 medical plan monthly rates are given in a table below. For the UHC PPO HSA 2250/4500 Plan and the Surest Zero-Deductible Plan the out-of-pocket maximums are also increasing.
- To help offset the medical cost increases we are introducing some benefit enhancements and added services:
- If you’re enrolled in an AI Fire medical plan (including the Surest Zero-Deductible Plan) you will have access to the UnitedHealthcare Concierge Care Team that provides personalized service if you need help understanding your medical, dental and vision plans, as well as many other benefits. - If you’re enrolled in an AI Fire medical plan, you can access the Calm Health mental wellness app for guided meditations, daily self-care, therapy referrals and more.
- Our prescription drug benefits are being updated to simplify the prescription drug coverage tiers, moving from four tiers to three. Learn more about your 2026 prescription drug benefits, provided through Optum Rx.
- Everyone enrolled in an AI Fire medical plan will be mailed new plan ID cards at home by January 1, 2026. The new cards will include both medical and prescription drug benefit information and updated contact information for 2026.
FSA and HSA Contributions
In 2026, you can make larger contributions to these accounts, giving you the opportunity to save more on taxes:
- Contribution limits have increased for all Flexible Spending Accounts (FSAs), including a substantial increase for the Dependent Day Care FSA.
- You can also contribute more to a Health Savings Account (HSA), another opportunity for increased savings.
The new contribution amounts are shown later on this page.
Other Enhancements for 2026
- The voluntary hospital indemnity plan will pay a $1,000 hospital admission benefit up to two times a year (increased from once per year). In addition, rates are decreasing for the hospital indemnity insurance and accident insurance plans.
- We’re switching the provider of our pet insurance plan from Pets Best to MetLife. MetLife offers extensive pet discounts, family plan coverage for up to three pets, expanded coverage for exotic pets, and more.
- Reminder: You’re eligible for additional employee discount programs, now that we’re a Blackstone company. Medical Plans
We are offering four Medical plan options, with each covering preventive care at 100% when you use an in-network provider:
- UnitedHealthcare PPO 1500/3000
- UnitedHealthcare PPO HSA 2250/4500
- UnitedHealthcare EPO HSA 4000/8000
- Surest Zero-Deductible plan

For the UHC PPO HSA 2250/4500 Plan, the in-network coinsurance that the plan pays for most covered charges will be 80% in 2026.
UHC Plan Overview
Each UnitedHealthcare plan has its own set of copays, deductibles, out-of-pocket maximums and coinsurance.
- The PPO HSA and EPO HSA are HSA-eligible plans, meaning that you can contribute to a Health Savings Account (HSA) and use it to pay for health care expenses with tax-free dollars. For 2026, you will be able to save up to $4,400 for self-only coverage and up to $8,750 if you cover any family members.
- For the PPO HSA Plan only, AI Fire will contribute $500 to your HSA when you enroll in individual coverage or $1,000 when you enroll in family coverage. (Note: This contribution from AI Fire counts toward your 2026 contribution limit of $4,400 for self-coverage, or $8,750 for family coverage.)
- With the two HSA plans, you do not have to meet a deductible before filling a prescription for preventive drugs if they are on the preventive medications list.
- For the PPO and PPO HSA plans, you can use in-network and out-of-network providers. Keep in mind, you will typically pay lower costs when you use in-network providers.
Surest Zero-Deductible Plan Overview
The Surest Zero-Deductible plan has a $0 deductible, with copays for each Medical service, treatment and prescription, except for preventive services which are covered at 100%. A key feature of the Surest plan is that you can use the Surest app to find out the cost of services in advance, so you can consider treatment options and compare costs from different providers. Note: You cannot use an HSA with the Surest plan.

Prescription Drug Overview
All four medical plans cover prescription drugs. Your cost for a covered drug depends on whether you fill your prescription from a retail location or via home delivery, and what tier the drug is in. For 2026, the tiers are:
- Tier 1: Generic, lowest-cost medications
- Tier 2: Preferred brand-name low-cost medications
- Tier 3: Non-preferred brand-name medications (lower-cost options may be available)

This program offers support if you’re diagnosed with prediabetes, diabetes or hypertension, and provides a simple and convenient way to manage chronic conditions with a participant portal that helps you manage your condition and track your progress.

If you’re dealing with a health situation such as recovering from a hospital stay, managing a health condition or expecting a baby, you may receive a call from a Compassionate Outreach Program nurse or advocate to offer additional support. Feeling stressed, anxious or struggling with coping skills? The AbleTo personalized, virtual behavioral coaching program includes coaching sessions with digital content, combined with activities using Cognitive Behavioral Therapy, to help you feel better soon. Learn more.
Real Appeal
Nearly seven out of 10 adults are considered overweight or obese, with serious effects on health care costs and long-term health. The year-long Real Appeal program offers you tools and support to help you lose weight, feel good and prevent weight-related health conditions. As part of your UnitedHealthcare Medical coverage, if you’re eligible the program will contact you about enrolling in and using the program.
Supplemental Hospital, Accident and Critical Illness for purchase
You may purchase these voluntary benefits to supplement your Medical plan coverage. They provide you with additional financial protection, by paying you a benefit if you become hospitalized, injured in an accident or are diagnosed with certain critical illnesses or health conditions.

Dental Overview
We offer two dental PPO plan options through UnitedHealthcare, the Base Plan and the Buy-Up Plan. Diagnostic/preventive services are covered at 100% under both plan options.
Dental Plans
Benefits under each plan are the same whether you use an in-network or out-of-network dentist. However, you will generally pay less when you use an in-network UnitedHealthcare dentist, because they agree to charge you a negotiated rate.
The Base option costs less for coverage and has a lower annual maximum benefit.
The Buy-Up option, which costs more for coverage, includes orthodontia benefits, higher coverage on certain procedures, the Flex Appeal® feature (enhanced benefits to protect the health of your teeth and gums) and Consumer MaxMultiplier® (which rewards you up to $600 for getting regular dental checkups).

Vision Overview
We offer Vision coverage through UnitedHealthcare. You can receive exams, lenses, frames, and contact lenses once per year. Children under 12 and pregnant or breastfeeding plan participants are eligible for two exams per year.
If you see an out-of-network provider, UnitedHealthcare will reimburse you up to a certain limit. The Vision plan also provides discounts on additional glasses, contact lenses, and hearing aids.

Note these updates for 2026:
- The maximum annual amount you can contribute to the Health Care or Limited Purpose FSA in 2026 has increased to $3,400.
- You will be able to roll over up to $680 in your Health Care or Limited Purpose FSA from 2026 into 2027.
- The maximum annual amount you can contribute to the Dependent Day Care FSA in 2026 has increased to $7,500 ($3,750 for married couples filing separately).
Commuter Benefits
